Welcome to the Forward Bank Data Settlement Online Claim Form submission page.

This Claim Form is to be used to apply for relief related to the Data Incident that Forward Bank discovered in September 2023 and publicly announced in November 2023 which potentially affected all individuals to whom Forward Bank sent notice. There are two types of benefits for which Class Members are eligible: (1) a cash payment of approximately $75 (subject to pro rata reduction or increase), and (2) reimbursement of outof- pocket losses or expenses that are reasonably traceable to the Data Incident, up to a maximum of $5,000.

To submit a Claim, you must have been identified as an individual whose Private Information was maintained on the Forward Bank database that was compromised during the Data Incident and received Notice of this Settlement with a Unique ID.

Please review the entire Claim Form. Failure to submit required documentation, if applicable, or to complete all necessary parts of the Claim Form, may result in denial of the Claim, delay its processing, or otherwise adversely affect the Claim.

TO BE VALID, THE CLAIM FORM MUST SUBMITTED NO LATER THAN JANUARY 8, 2025.
